Appointments:
Initial new patient appointments are 1hour.
Follow-up appointments will vary in length, approximately 30 minutes.
Forms:
Please download and complete the new patient forms and bring them with you to your first appointment. You may also call the office and we will be happy to mail or fax all necessary forms to you. The forms are in PDF format and require Adobe Reader. If you do not have Adobe Reader, you may download it here.
The following forms are required for all new patients:

Fees:
Fees are based on the length of appointment and complexity. Please request a copy of our fee schedule. We accept payment by cash, check or major credit cards for all treatment, which is due at the time of service. Insurance co-pays are also due at the time of service.
Insurance:
Please let us know if you need help determining if your insurance company covers naturopathic services. If your insurance provider does cover our services, we will submit your claim directly to them.
